Jason Chorak (Jason J. Chorak) was born on September 23, 1974 in Seattle, Washington. After going to high school at Vashon (WA), Chorak attended the University of Washington. Chorak made his professional debut in the NFL in 1998 with the Indianapolis Colts. He played for the Indianapolis Colts for his entire 1 year career.
