Dave Middendorf (David Warren Middendorf) was born on November 23, 1945 in Seattle, Washington. After going to high school at Ingraham (Seattle, WA), Middendorf attended Washington State Universtiy. Middendorf made his professional debut in the AFL in 1968 with the Cincinnati Bengals. He played in the AFL for 2 years, playing for the Cincinnati Bengals the entire time. Middendorf also played in the NFL for 1 year, playing for the New York Jets the entire time.
